This includes items such as co-payment and deductible. If you have no "primary medical insurance", you will need to provide an affidavit to the insurance company at the time of loss. If you have "primary medical insurance" you will need to provide a disposition of your claim with your primary insurance and supplemental insurance carriers upon filing a claim.
